Fuse code and meta ref updates in NoteDb

JGit now supports atomic multi-ref BatchRefUpdates natively in
FileRepository, so we can eliminate the distinction between fused and
unfused NoteDb modes. Now, disableChangeReviewDb=true means NoteDb is
fully enabled.
