OpenStack Block Storage (Cinder)

Current Kaminario K2 driver only supports iSCSI multipathing using discovery (sendtargets), which in some cases may be undesirable, since a broken primary path could prevent an attachment. This code adds the possibility of not using discovery mode and return all connection information when `initialize_connection` is called, which should provide increased reliability.
