Roberto Polli 2823fcafdc Replace LBaaS with LBaaSv2.

Since Mitaka, OS::Neutron::LBaaS::LoadBalancer replaces
  OS::Neutron::LoadBalancer. The patch replaces both
  heat_template_version and OS::Neutron::LoadBalancer.
