Ronald Bradford 5e677dc232 Fix coverage configuration and execution

A number of configuration errors prevent the successful creation of code
coverage. This corrects the .coveragerc source/omit setup and the tox
package name generation.

The Oslo context library has helpers to maintain useful information about a request context. The request context is usually populated in the WSGI pipeline and used by various modules such as logging.
