Deployment of a managed Ceph cluster using puppet-ceph
is not supported from the Pike release. From Queens it
is not supported use of puppet-ceph when using an
external Ceph cluster either.
This change removes the old templates necessary to
support deployment of Ceph via puppet-ceph.
