Remove footer blocks from included documentation pages

The 'database-setup' and 'config-login-register' pages are only used as
includes in other pages, rather than being linked as standalone pages,
however they still have the footer blocks.

This causes the 'Part of Gerrit Code Review' section to be rendered in
the middle of the pages in which they are included. For example it can
be seen above 'Initialize the Site' on the installation page [1].

Remove the unnecessry blocks.
