Ryan Williams a19a7f3858 Removed Green_fileobject and moved its contents to GreenPipe. Removed higher_order_send and higher_order_recv, integrating their contents with the appropriate methods on GreenSocket and GreenPipe. Fixed up callsites that were depending on various aspects of Green_fileobject's behavior. Added a backdoor unit test.
