Kurt Griffiths fcde52eff3 test: Migrate to combined test coverage ...

Minimize the use of "pragma: no cover" by combining coverage results
across multiple runs. Update CONTRIBUTING.md with instructions for
checking coverage, etc. before submitting a PR.
