follows cycle-with-intermediary release model

Adds a note to the specification to reflect that ironic follows the
cycle-with-intermediary release model, not the independent
release model.

(At the time, I don't think the cycle-with-intermediary release
model had been defined.)

Also clean up formatting of the references section.
