[glossary] Consolodate project codenames [ceilometer]

- Combine "Telemetry service" entry with "ceilometer" entry
  - Rename "Telemetry service" to "Telemetry service (telemetry)"
  - Link "ceilometer" to "Telemetry service (telemetry)"
  - Make sure description matches project.yaml
  - Add terms for aodh, gnocchi and panko as telemetry components
- Resolve glossary references
