Updates for Kilo release
Many small updates to various sections to: * remove/reword/redirect old pre-icehouse cruft * update links from Juno to kilo (eg config reference) * generally improve anything that looked dated Change-Id: I26618be8527cccc1cbb7811272613da7d410fe50
